Job Summary
Homeside Rehab, a leading provider of home-based rehabilitation and behavioral health services, is seeking Licensed Mental Health Counselors (LMHCs) and Licensed Clinical Social Workers (LCSWs) to provide high-quality, evidence-based mental health services to pediatric and adolescent patients in the Bronx and Queens, New York. As a Licensed Therapist, you will conduct comprehensive mental health assessments, develop individualized treatment plans, and provide individual, family, and caregiver counseling using evidence-based therapeutic approaches.
Responsibilities
- Conduct comprehensive mental health assessments, including psychosocial evaluations and diagnostic assessments.
- Develop individualized treatment plans tailored to each patient's clinical needs and goals.
- Provide individual, family, and caregiver counseling using evidence-based therapeutic approaches, including CBT, Solution-Focused Therapy, Mindfulness-Based Interventions, Trauma-Informed Care, and other evidence-based treatment modalities.
- Monitor patient progress and adjust treatment plans as appropriate.
- Collaborate with physicians, rehabilitation professionals, social workers, and other members of the interdisciplinary care team.
- Maintain accurate, timely, and compliant clinical documentation, including evaluations, treatment plans, and progress notes.
- Educate patients and families regarding treatment goals and coping strategies.
- Participate in clinical meetings, supervision, and continuing education opportunities.
- Maintain compliance with all federal, state, agency, and ethical standards.
Qualifications
- Master's degree or higher in Mental Health Counseling, Social Work, Psychology, or a related behavioral health field.
- Active New York State LMHC or LCSW license in good standing.
- Experience providing therapy services to children and adolescents.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently in patients' homes while maintaining professional standards.
