Position Overview
We are seeking a compassionate and motivated Post-Acute Care Physician (MD/DO) to provide on-site primary care services within skilled nursing facilities throughout the Tampa area.

Clinical Responsibilities
- Provide comprehensive primary care services to residents in skilled nursing facilities
- Evaluate, diagnose, and manage acute and chronic medical conditions
- Conduct admissions, follow-up visits, and medically necessary evaluations
- Assess and manage residents experiencing changes in condition
- Collaborate with nursing staff, therapists, specialists, and facility leadership
- Supervise and support NP/PA clinicians as appropriate
- Participate in quality and patient care initiatives
After-Hours Coverage
- Participate in a structured and manageable call rotation
- Address urgent clinical concerns through telehealth services
- Provider-delivered telehealth encounters are billable
- Supported processes designed to maintain reasonable call volume

Pay-Per-Encounter Model

Qualifications
- MD or DO degree required
- Board Certified or Board Eligible in:
- Internal Medicine
- Family Medicine
- Geriatrics
- Emergency Medicine
- Active Florida medical license (or ability to obtain)
- DEA registration (or ability to obtain)
- Interest in post-acute care, skilled nursing medicine, and collaborative patient care
